Reevaluating SGLT2i Guidelines: Study Finds No Major DKA Risk Post-Surgery
21 Feb 2025 • New research challenges the current recommendation to withhold sodium-glucose cotransporter 2 inhibitors (SGLT2i) before surgery.
Analyzing data from over 34,000 patients with type 2 diabetes undergoing emergency surgery, researchers found no significant increase in postoperative diabetic ketoacidosis (DKA) risk among SGLT2i users. While unadjusted DKA incidence was slightly higher in users than non-users (4.9% vs. 3.5%), adjusted analysis showed a negligible difference (3.8% vs. 3.5).
These findings suggest that current guidelines recommending a three-day preoperative SGLT2i hold may warrant reconsideration, potentially improving perioperative glycemic management.
