Critical Window: Best Timing for EVT in Large Ischemic Strokes Revealed
12 Dec 2024 • A recent analysis of the Endovascular Therapy in Acute Anterior Circulation Large Vessel Occlusive Patients trial found that EVT significantly improved favorable outcomes when administered within 6-12 hours of symptom onset.
- Compared to medical management (MM), EVT led to better functional outcomes (modified Rankin Scale 0–3) in those treated within 6 hours (44.4% vs. 29.9%) and between 6–12 hours (45.7% vs. 29.6%).
- However, benefits diminished beyond 12 hours, with no significant advantage after 13 hours and 22 minutes.
These findings strengthen the benefit of EVT initiated within 13 hours and 22 minutes after symptom onset compared with MM alone in patients with large ischemic stroke, but EVT should not be withheld for patients presenting beyond 13 hours and 22 minutes.
