Severe COPD Predicts Poor Survival and Complications After Lung Cancer Surgery

4 Sept 2025 • In non-small cell lung cancer (NSCLC) patients undergoing anatomical resection, severe COPD (GOLD III–IV) was linked to higher postoperative complications, including prolonged ventilation, air leaks, and arrhythmias, compared with mild/moderate COPD (GOLD I–II).

Median overall survival was significantly shorter in severe COPD (43.7 vs. 85.3 months).

Independent predictors of reduced survival included severe COPD (HR 1.6), advanced tumor stage (pT3–4, HR 1.3), nodal involvement (pN1–2, HR 2.1), elevated CRP, and low VO₂max.

These findings underscore COPD severity as a key determinant of perioperative risk and long-term outcomes in operable NSCLC.
