Intervention Yields Better Hemorrhage-Free Survival Than Conservative Approach in AVMs
18 Nov 2025 • A study involving 1,770 patients found that early interventional treatment for unruptured cerebral arteriovenous malformations (AVMs) greatly improved hemorrhage-free survival.
Researchers comparing conservative management (medical therapy and observation) with interventional treatment (microsurgical resection, stereotactic radiosurgery, endovascular embolization, or combinations) initiated within six months of diagnosis found that at five years, hemorrhage-free survival was 96.2% with intervention compared to 89.0% with conservative therapy (HR 0.44). The benefit lasted for 10 years (HR 0.56). However, the benefit was not seen in high-grade AVMs or diffuse nidus patterns. Microsurgical resection was found to be the most effective treatment.
Interventional treatment may provide a significant long-term safety benefit for carefully selected patients.
