One-Year Death Risk Skyrockets with HF, CKD, and ASCVD Multimorbidity

20 Nov 2025 • Adults aged 75 years and older with heart failure (HF), chronic kidney disease (CKD), or atherosclerotic cardiovascular disease (ASCVD) showed a markedly elevated one-year mortality risk, with the hazard rising sharply when these conditions coexist.

Mortality was nearly ninefold higher with ASCVD alone (HR 8.42) and similarly increased with HF alone (HR 9.19), while CKD increased mortality more than fivefold (HR 5.35) versus controls. The

combination of HF, CKD, and ASCVD carried the greatest threat, driving a 14-fold higher likelihood of death within one year (HR 14.18), alongside higher cardiovascular mortality, more HF hospitalizations, and faster progression to end-stage kidney disease.

The findings reinforce the need for tightly coordinated cardiovascular–renal care to reduce near-term mortality and clinical deterioration in multimorbid older adults.
