AHA Flags Urgent Gaps - and Clear Do’s - in Managing Stroke During and After Pregnancy

3 Feb 2026 • In a new scientific statement, the American Heart Association highlights that stroke, though rare, remains a life-threatening complication of pregnancy and postpartum, with nearly half of pregnancy-associated stroke hospitalizations linked to hypertensive disorders.

Pregnancy should not delay evidence-based treatments for acute stroke.

  • Computed tomography, computed tomography angiography, and magnetic resonance imaging without contrast are all safe neuroimaging modalities for rapid evaluation of pregnant patients with acute stroke symptoms.
  • Maternal stabilization should take precedence over delivery decisions.
  • Vaginal delivery after stroke is preferred when feasible because it avoids the surgical risks and hemodynamic stress associated with cesarean delivery.
  • Survivors require coordinated multidisciplinary rehabilitation to address unique postpartum challenges such as infant care and breastfeeding.

The AHA emphasizes an urgent need for inclusive research to refine risk assessment, expand treatment options, and improve maternal outcomes.
