Muscle Mass Modifies Cardiovascular Risk from Intensive Blood Pressure Lowering

30 Jun 2025 • Intensive blood pressure (BP) control may be harmful in patients with low muscle mass.

While those with normal muscle mass experienced significant cardiovascular (CV) benefits (HR 0.71; absolute risk reduction: 5.2 events per 1,000 person-years), individuals with low muscle mass faced an increased risk of primary CV events (HR 1.72; absolute risk increase: 11.1 events per 1,000 person-years), with excess risk becoming apparent after 3.4 years.

The cognitive benefits of intensive BP lowering were consistent across muscle mass groups. The findings underscore the importance of tailoring BP targets in frail or sarcopenic populations to avoid unintended cardiovascular harm.
