Low Perfusion Pressure Linked to Higher AKI Risk After Cardiac Surgery

18 Jul 2025 • In post–cardiac surgery patients, tissue perfusion pressure (TPP = MAP − critical closing pressure) outperformed mean arterial pressure (MAP) in predicting acute kidney injury (AKI).

Key Findings:

  • AKI occurred in 17.6%, tripling major morbidity (45.1% vs 14.2%) and raising mortality nearly eightfold (15.8% vs 2.0%).
  • TPP < 38 mmHg raised AKI risk (OR 1.75; 1.69 with vasopressors).

A high-risk phenotype (TPP 35.6 mmHg) saw ~1 in 3 develop AKI, highlighting TPP as a key hemodynamic target.
