Elevated SII Predicts Severe Acute Ischemic Stroke: A Robust Biomarker for Risk Stratification

19 Jun 2025 • A study of 1,723 acute ischemic stroke (AIS) patients identified the Systemic Immune-Inflammation Index (SII) as a strong predictor of stroke severity.

Key Findings:

  • Higher SII levels were independently linked to severe strokes (NIHSS ≥8), with each 100-unit increase in SII raising the odds by 4% (OR: 1.04).
  • The strongest association was observed in the SII/100 range of 2.4–7.8.
  • Patients in the highest SII quartile had a 3.46-fold higher risk of severe stroke versus the lowest.

The link held across all subgroups, supporting SII's utility in AIS risk stratification.
