Intravascular Imaging-Guided PCI Improves Clinical Outcomes for Chronic Total Occlusions
13 Mar 2025 • A recent meta-analysis of five randomized trials involving 1,296 patients validated the superior clinical outcomes of intravascular imaging (IVI)-guided percutaneous coronary intervention (PCI) over angiography-guided PCI for chronic total occlusions (CTOs).
IVI-guided PCI significantly reduced major adverse cardiac events by 45% (7.2% vs. 13%; RR 0.55) and target-vessel revascularization rates by 48% (3.1% vs. 6.7%; RR 0.52). While no notable differences were observed for myocardial infarction or mortality, the results underscore the potential benefits of IVI-guided PCI in CTO treatment.
