Albuminuria Strongly Predicts Cardiorenal Risk in Hypertension—but Is Rarely Measured

22 May 2025 • Analyzing over 144,000 patients who newly started antihypertensive therapy, a striking 80% had no urinary albumin-to-creatinine ratio (uACR) measurement at treatment initiation, despite uACR’s strong predictive value for cardiorenal outcomes.

Patients with macroalbuminuria faced a significantly higher 2-year risk of major adverse cardiovascular events (MACE: 5.3%, HR 2.02), heart failure (HR 1.99), a 40% decline in eGFR (HR 4.81), and end-stage kidney disease (HR 4.52) compared to those with normoalbuminuria.

The findings spotlight uACR as a powerful yet underused tool in hypertensive care and underscore the urgent need for routine albuminuria screening.
