Routine IOC Unnecessary for Isolated CBD Dilation During Cholecystectomy: Outcomes Unaffected
12 Jun 2025 • A new study questions the routine use of intraoperative cholangiogram (IOC) during laparoscopic cholecystectomy for isolated common bile duct (CBD) dilation on ultrasound.
Among patients with isolated CBD dilation (≥0.6 cm + 1 mm), there was no increased risk of choledocholithiasis. Ultrasound showed modest diagnostic accuracy—sensitivity 80%, specificity 25.5%, and positive predictive value just 17.4%.
Clinical outcomes and readmission rates were unaffected, suggesting that isolated CBD dilation may not warrant routine IOC.
