Stroke Stratification in AF: Women Risk Rises with CHA₂DS₂-VA Score ≥2
3 Jun 2025 • A Taiwanese study of nearly 5 lakh atrial fibrillation (AF) patients found that women consistently had higher ischemic stroke risk than men, with incidence rate ratios ranging from 1.13 to 1.26.
Stroke risk was especially elevated in women with higher CHA₂DS₂-VA scores (2–8; IRR 1.04–1.11) than those with lower scores (0–1).
The traditional CHA₂DS₂-VASc score, which includes female sex, surpassed the sex-less CHA₂DS₂-VA score in predicting stroke events in net reclassification index, particularly in earlier years.
Findings suggest that female sex remains a relevant stroke risk modifier in Asian AF patients, supporting the continued use of CHA₂DS₂-VASc for risk stratification.
