Low HBsAg Levels Signal Negligible HCC Risk in Inactive Hepatitis B
20 Jun 2025 • In a long-term study of over 2,600 inactive chronic hepatitis B patients, those with hepatitis B surface antigen (HBsAg) <100 IU/mL had a very low annual hepatocellular carcinoma (HCC) incidence of 0.08% over 26.3 years, significantly lower than those with HBsAg ≥100 IU/mL (HR 0.35).
This risk was below the threshold for routine surveillance, even in older adults.
Combining HBsAg <100 IU/mL with normal ALT may help identify low-risk patients without HBV DNA testing, supporting refined monitoring and partial HBV cure strategies.
