High-Risk ECOPD: Home Oxygen, Neutrophils, and Acidemia Predict Treatment Failure
10 Jul 2025 • In a study of hospitalized exacerbation of chronic obstructive pulmonary disease (ECOPD) patients, 17% experienced treatment failure, defined as therapy escalation or in-hospital death.
Key predictors included home oxygen therapy (OR 5.34), neutrophil count ≥7000/μL (OR 3.55), and acidemia (OR 3.13). Among those on narrow-spectrum antibiotics, failure was significantly higher in patients receiving home oxygen.
The findings underscore the need for early risk identification and tailored treatment, including broader antimicrobial coverage for high-risk patients.
