Early FDC Linked to Hemorrhagic Transformation in Stroke Bridging Therapy
23 May 2025 • Early fibrinogen depletion coagulopathy (FDC) occurs in 18.2% of acute ischemic stroke patients receiving bridging therapy (intravenous thrombolysis (IVT) + endovascular treatment) and significantly raises the risk of hemorrhagic complications.
FDC was strongly linked to IVT use and independently predicted any hemorrhagic transformation (HT) (aOR 2.33), symptomatic HT (aOR 3.35), parenchymal hematomas (aOR 2.87), and 3-month mortality (aOR 3.82).
None of the patients treated with tenecteplase developed FDC, compared to 39.3% of those given alteplase, highlighting a possible safety edge for tenecteplase.
